Here is a sample of the of the 360 Peer Evaluation form. I want you to create peer evaluation form in excel based on the attached traits (include all the traits). The rating is from 1-5, kindly add the 5 rating with its description as a drop list (kindly note that each trait has different rating description. At the end add total of rating (automatically sum). Add comment box at the end of the form Write the title on the top of the form and the following information: Date: Employee Initials: Evaluator Initials: Please be objective and tick (only one choice), don’t miss any of the traits. If you did experience any of the traits with your colleague, kindly tick N/A. Support the evaluation with comments, especially when you rate your colleague 3 and less.
Kindly adjust the margins and the page because this form will be printed. Try to create the form in 2 pages, not more.
