Nursing is an essential component of any health care organization, and nursing leaders must understand the role of nursing and nursing leadership within the total organization. Understanding basic organizational structure, mission, vision, philosophy, and values will better prepare nurse leaders to improve quality and patient outcomes.

Preparation

Use the Suggested Resources for this assessment to study systems theory, SWOT analysis, and shared governance teams, as background for an Impact Report to Senior Leadership. The report includes a SWOT analysis. The SWOT Analysis Template is linked in Required Resources.

• Study a facility in your area and examine its Web site and any available public documents to aid you in completing this assessment. You may wish to examine an organization where you work, where you have worked in the past, or where you did your clinical work.
• Do not name the organization in your report; instead, identify the type of facility and location. Example: A rehabilitation center in a midsize Midwestern city.

Use the following scenario as the basis for your report:

Scenario
The unit-based Shared Governance Council of the organization where you work has identified a major nursing-related challenge (patient/staff safety, confidentiality, management/staff conflict, poor patient satisfaction survey scores, or nursing staff shortages) within your health care organization.
In an unusual move, your system administrator is considering creating a new nurse leadership position that would have the responsibility and authority to address this specific issue. As a member of the shared governance council’s subcommittee, you have been asked to write the committee’s final report.
 

Deliverable: Impact Report to Senior Leadership

Write a 4–6 page impact report to senior leadership that identifies the challenge, analyzes how it affects the organization from a nursing perspective, and details the new position.

Construct the report with the following headings:

The Nursing Challenge:

• Identify the nursing challenge and its impact from a nursing perspective. Choose from:      
  • Patient/staff safety, confidentiality, management/staff conflict, poor patient-satisfaction survey scores, or nursing staff shortages.

The System/Organization:

• Explain how the nursing challenge creates a gap or conflict between the organization’s statements and practice.      
  • Identify the organization type—that is, specialty hospital, teaching hospital, major health care system, et cetera, and summarize the organizational structure, its mission, vision, and philosophy statements.
  • Use systems theory and systems thinking to explain the gap or conflict.

SWOT Analysis:

Use the SWOT Analysis Template linked in Required Resources and include the SWOT analysis table in your paper.

• Identify organizational factors that impact the situation using a SWOT analysis.      
  • For example, a budget cut may cause short staffing, which relates directly to the problem.
• What factors within the system may facilitate a solution for this problem?

The Position:

• Assess how the new nurse leader position will have power and influence and impact patient outcomes.
• How will this position affect change within the organization?
• Identify key leadership skills, knowledge, or abilities required for the position.

Additional Requirements

• Written communication: Written communication should be free of errors that detract from the overall message.
• APA formatting: Resources and in-text citations should be formatted according to current APA style and formatting.
• Length: The report should be 4–6 pages in content length. Include a separate title page and a separate reference page.
• Font and font size: Times New Roman, 12 point, double-spaced.
• Number of resources: Use a minimum of three peer-reviewed resources.

Briefly summarize your project:  How was the assessment completed? Who was your target audience( patient at the rehabilitation) and how many were in the audience? What was your topic?( fall prevention)  How did you determine what your topic should be (justification)?  When( march 1st 2019) and where( sava senior care, Glenburnie MD).  did you present the community education project?  Who gave you permission to present at the location,( Ms Carol Brown) was the permission form completed? yes.   How did you advertise your presentation?  ( nurses, patient tech, physical therapist and occupational therapist) What educational materials did you use or provide to the audience? (Provide ID of yourself and action picture for proof)

Evaluation 

Explain how you would evaluate whether the efforts to improve the health concern were effective. Include in your explanation the tools you might use to do this evaluation. Also, critically appraise your performance of the activity/project for this population. How did the population respond? What aspects were you most satisfied/least satisfied with? What went well (at least 3 things)? What changes would you make in the future (at least 2 things)? 

Conclusion 

For nurses, reflection is integral to higher-level thinking. You should be able to critically analyze scenarios and possible outcomes as opposed to simply asking the right questions. Acute perception of the entire situation will be obtained because you are open to dealing with the contradictions that may challenge your initial bias. Reflect on how your perspective of the community’s health and the national, state, and local efforts toward a healthier population has changed as a result of your fieldwork. Discuss the health of the target community population compared to national, state, local health findings. Discuss how the interventions used would impact at the local, state, and national level (include information on your chosen Healthy People 2020 goals). Finally, what impact did your project have on your target population?

1. Please submit a recorded presentation of between 7 and 9 minutes that is a reflection of the project based on a full representation of all three parts above. You may video yourself discussing the project or you may create a PowerPoint presentation and record your voice to the presentation (using screencast-o-matic.com). The presentation should include at least two scholarly sources other than provided materials. Please be sure to provide a photo of you at the location where you completed your education project and your ID again. You may submit your Reference page and photos in a separate file in the dropbox or insert them into the PowerPoint.

Part two:

Assignment Description: The final community assessment paper will be submitted in Module 8; therefore, there is no paper to be submitted this week for Mod 7 – only the updated cumulative hour log.  This week please just add the hours you spend completing the community assessment to your Project Hours Log and submit. The Log is all that is required this week for submission (You should have approximately 2-4 hours logged for the geriatric assessment from Mod 2 and now an additional 10-14 hours for Mod 7 portion of the community assessment.Any research that is completed or people who are interviewed should be included in the week 7 log. Please incorporate not only on-line research, but conversational/personal data, as well.

In this module, you will complete a full assessment of your own community.  (You will be using this for your Community Health II course as you plan, implement, and evaluate an education teaching project within your community designed to address an identified health concern of your chosen population.) For this week, in the Module 7 Overview there are several areas which must be assessed for a fully developed assessment. Review this section thoroughly.

• First is to Defining the Community; answer all questions noted in this section in the Overview. Your community can be a pediatric, adolescence, young adults, adult, elderly or a combination. Or it can be a specific ethnic group or a group such as truck drivers, farmers, or campers. Be sure to define cultural context of the population, demographics such as age, race, education, birth/death rates, life expectancy, insurance status, and other pertinent details. Connect community context and demographics with Healthy People 2020 objectives and identify a health concern.I know the assignment instructions say to select a specific population, but for an easier transition into N493, remain very broad when doing your research this week. The broader, the better! Focus on your county, city or town. Having a broad community assessment will serve you better as you go into N493 (community health 2). In that course you will pick a more specific population, like children, adolescents or adults for the primary prevention project.
• After the community has been defined, the next phase is Assessment. In the Overview section there are several resources and methods that can be used to gather and generate data. These items serve as a starting point for data collection. Please be sure to use this list to complete a comprehensive assessment of your community. These include:
